1. All associated lab work and diagnostic procedures are included in the fee quoted for this particular service (i.e. Hematocrit, Sonogram, Office Visits, etc.). 
2. Established patients are any patients who have been seen in the past 36 months.  All other patients are considered "New" patients for billing purposes.
3. This procedure is subject to an additional office visit fee if not performed in conjunction with any other service.
